This module provides guidance on developing comprehensive care plans in accordance with Kansas regulations, emphasizing the roles of SSDs and Activities Directors in the interdisciplinary team.

Regulatory Framework

According to Kansas Administrative Regulations:

  • Interdisciplinary Assessment: A comprehensive care plan must be developed from an interdisciplinary assessment, including medical, nursing, dietary, activities, and psychosocial evaluations. K.A.R. 28-34-29a
  • Social Services Designee: Facilities with 120 beds or fewer must employ a social services designee to address residents' psychosocial needs. K.A.R. 28-39-153

Roles and Responsibilities

Social Services Designee (SSD):

  • Conduct psychosocial assessments.
  • Participate in care plan meetings.
  • Develop interventions to address social and emotional needs.

Activities Director:

  • Assess residents' activity preferences and capabilities.
  • Design and implement activity programs.
  • Collaborate with the care team to integrate activities into the care plan.

Sample Care Plan

Identified Needs:

  • Feelings of social isolation.
  • Limited participation in group activities.

Goals:

  • Increase social interaction to at least three group activities per week within the next month.
  • Enhance sense of belonging and reduce feelings of isolation.

Interventions:

  • SSD: Schedule weekly one-on-one counseling sessions to discuss feelings and encourage participation.
  • Activities Director: Introduce resident to small group activities aligned with his interests, such as a book club or gardening group.

Evaluation: Monitor attendance and engagement in activities weekly. Adjust interventions as needed based on resident feedback and participation levels.
