Social Service Designee / Activities Director

A Social Service Designee will interview residents to ensure their physical, psychological, and social needs of the residents are being met within a facility, and implement personalized plans of care which are designed to improve or restore Activities of Daily Living.

The Activities Director is responsible for creating an ongoing program of activities designed to meet the interests and the physical, mental, and psycho-social needs of each resident in accordance with the comprehensive assessment.
